People received safe care and support from staff who understood their needs. Staff knew how to safely support people around their individual risks and people’s safety were assessed to ensure they were supported to remain as safe as possible. There were sufficient numbers of suitably trained staff to meet people’s needs. Staff knew how to escalate any concerns and understood their responsibilities in relation to safeguarding.

Learning culture

Score: 3

People felt staff knew how to support them safely. People told us they were confident in the ability of staff to support them. One person told us, "They will go the extra mile and carry out tasks like emptying the bin and cleaning the bath and shower which is always done with compassion and dignity."

Staff told us the manager and office staff deal with accidents and incidents and support them with any learning. There was a system in place in which to record incidents. The registered manager reviewed incidents and took action in response to them, noting lessons learnt and next steps for staff.

Systems were in place to learn from accidents and incidents. We saw that lessons learnt were completed following accidents, incidents, complaints and safeguarding concerns. There were robust monitoring systems in place to ensure that lessons were learnt from incidents.

Safe systems, pathways and transitions

People felt staff knew how to support them safely and were confident in the ability of staff to support them. People told us that they received the care they wanted and that staff were good at their jobs. One relative said, " Staff were brilliant and they knew mum well and couldn’t fault them at all. They were so lovely and patient, talking to her, speak to her, communicating, they would always tell her who was there."

Staff said they were given information about people prior to commencing care. This enabled them to understand why a person may need care. Staff demonstrated an understanding of people’s individual needs and preferences and spoke positively about the people they supported. Staff told us they received good support from other healthcare professionals and that guidance from professionals was shared and followed.

The local authority told us the provider was open and had a good working relationship working with them with ongoing improvements and enabling the provider to improve the care and support they deliver.

The provider had processes in place to ensure safe system, pathways and transitions were maintained. This included an initial needs assessment at the start of a service being provided. The care coordinator or the registered manager carried out assessments with a person face to face.

Safeguarding

People were protected from the risk of abuse and told us they felt safe using the service. One relative told us, "Adada is the best care and support provider that [Name] has ever had, and they make things a lot easier for them. They are always friendly and supportive and have gotten to know [Name] very well."

Staff were able to describe what they would do if they suspected abuse was taking place. Staff confirmed they had received safeguarding training. Staff told us management would take appropriate action if concerns were raised. The registered manager was aware of their role and responsibilities to safeguard people from harm and abuse.

Safeguarding procedures were in place and staff had access to policies and procedures if they needed them. The service had recently had several safeguarding events, and they were effectively reported and investigated. Staff had completed safeguarding training and understood how to report any concerns. When incidents happened, they were investigated, reported to the local authority and CQC were informed when appropriate.

Safe and effective staffing

We received mixed feedback about consistent staff turning up to deliver the care. However, all the people and relatives we spoke to were positive about the staff even if they were not their regular staff. One relative said, "We have no problem with the company or carers, they arrive on time or inform us if they have an emergency at the previous destination and will be with mum as soon as they can."

The registered manager and care coordinators told us that consideration is given to the geographical area in which people live to ensure staff can complete their calls. This supports care calls being delivered on time. Most staff confirmed this to be the case, however some staff told us they do not have enough time between calls, and this had increased in the past few months.

The majority of recruitment files contained all the needed information, however we found that there was a gap in the recruitment process, and we found one staff member had been recruited and their work history had not been explored properly. The provider made immediate changes to the processes. There were systems in place to plan the rota to ensure people’s hours were met. Staff had the training they needed to provide support to people, this included a mixture of face to face and online learning. Training included areas such as safeguarding people, understanding learning difficulties and disabilities, understanding equality and diversity and pressure ulcer care. New staff shadowed more experienced staff to learn about people and their needs.
