Systems were in place to keep people safe from the risk of abuse and harm and staff demonstrated good knowledge and confidence about what to do if they had any concerns about the safety of people they supported. We found the service was working within the principles of the MCA and if needed, appropriate legal authorisations were in place to deprive a person of their liberty. Any conditions related to DoLS authorisations were being met. There were sufficient staff on duty to meet people’s needs and to deliver good quality care and support. Staff were caring and compassionate in their support for people. The environment was clean and infection prevention and control measures promoted people's safety.

Learning culture

There was a culture of learning and safety in the service. The provider had learnt lessons since the last inspection and developed the staff team to be able to better manage situations. The service had put things right, learned and improved the service. Lessons learned are discussed at staff meetings and staff are actively encouraged to discuss any contributions. The registered manager was aware of their duty of candour and took appropriate actions when things went wrong. There was a culture of openness and transparency. Accidents and incidents were monitored and analysed to mitigate future occurrences.

Staff told us the home had improved since last inspection and gave positive feedback about working at the home. One person told us, "The registered manager is a very approachable person and is always there to coach us through anything that crops up in a professional manner." Another member of staff told us, "Neville lodge is a good environment for people with sensory needs. It is a comfortable space which helps create balance in response to each individuals needs."

People who lived in Neville Lodge felt safe and were supported to understand and manage any risks. A family member told us, "[Relative] is definitely safe at Neville Lodge. It's made a big difference to their life and they now engage well with staff. [The registered manager] is very proactive and approachable."

Safe systems, pathways and transitions

Safety and continuity of care was a priority and the provider demonstrated a collaborative and joined-up approach which provided people with good outcomes. Health action plans were in place and were person centred and easy to read. They contained information about pain and consent, health screening, and medicines. Following last inspection outcome of inadequate, the management team had to make changes to the service user group as they were no longer able to meet their needs and facilitate their care because this was impacting on other people who were living at the service. The registered manager explained how they did this in a way so that safety and continuity of care was maintained as much as possible and people were involved in decisions about their care.

Staff told us the home had improved since last inspection and gave positive feedback about working at the home. One person told us, "The registered manager is a very approachable person and is always there to coach us through anything that crops up in a professional manner." Another member of staff told us, "Neville lodge is a good environment for people with sensory needs. It is a comfortable space which helps create balance in response to each individuals needs."

Professionals involved in people's care were complimentary about the service and told us the management team were proactive and managed risks effectively. One professional told us, "The staff team are very approachable and communicate well with the community mental health team (CMHT). They have organised meetings which are inclusive of all stakeholders and person centred with the person being included in all aspects of their care and treatment. People are encouraged and facilitated to plan their care and treatment." Another professional told us, "Staff have always been involved in review processes and have demonstrated the close relationships they have developed with people they support. Staff always attend and represent the best interests of people during other meetings with professionals too, such as reviews and medical appointments."

Relatives we spoke with were complimentary about the service and the care and support their family member received. They told us support was appropriate and that staff facilitated people to be at the centre of their own care and as independent as possible.

Staff complete safeguarding reviews and assessments and discussed at team meetings. Mental Capacity Act (MCA) and DoLS assessments are completed face to face with people. There is safeguarding log in place which records date, response and date closed. Lessons are learned as a result.

The management team and staff understood the safeguarding procedure and took appropriate actions to keep people safe. There are safeguarding policies and procedures in place.

Involving people to manage risks

Support plans completed were in-depth and detailed and were supported by a risk matrix to identify risks in relation to that specific care plan. Support plans state whether a risk or a need has been identified and are then written in this context. This ensures that support plans accurately reflect the support that people need.

Risk assessments in regards to building and equipment safety were kept up to date and well maintained.

The service was designed to meet people's needs and cater for different preferences. People had access to a games room, lounges, quiet areas and communal dining areas.

Staff were supported and trained to carry out their role and responsibilities well. Staff were recruited safely and people were involved in selecting staff to work in their home.

Staff felt the registered manager and the management team were approachable and that they supported staff to develop within the organisation.

Infection prevention and control

Infection prevention and control measures promoted people's safety. Staff had received training about how to prevent infections and how to use personal protective equipment (PPE).

Relatives we spoke with were complimentary about the service and the care and support their family member received. They told us support was appropriate and that staff facilitated people to be at the centre of their own care and as independent as possible.

A tour of the home confirmed the service and environment was clean and well maintained. People were encouraged to take responsibility for cleaning in their rooms and communal areas.

Medicines optimisation

Medicines were managed in a safe way and people received their medication safely. Medicine management within the home reflected current and relevant best practice and professional guidance.
