Well-led – this means we looked for evidence that service leadership, management and governance assured high-quality, person-centred care; supported learning and innovation; and promoted an open, fair culture. This key question has been rated requires improvement. This meant the service management and leadership was inconsistent. Leaders and the culture they created did not always support the delivery of high-quality, person-centred care. We found breaches of regulation during this inspection.

Capable, compassionate and inclusive leaders

Staff told us they felt well supported by their management team. In our observations, we observed the manager's approach with people, relatives and staff being inclusive and respectful.

Governance, management and sustainability

Score: 2

Staff were well supported by the manager. No one had any concerns about the management team.

Although there had been improvements since last inspection in how the service was managed, we continued to find records of care were not complete and quality assurance systems were not fully effective. We found quality assurance systems in place had not always been effective in ensuring areas for improvement were identified and addressed. Care plan audits had been completed on the care plans reviewed, but the issues we identified during this inspection with records not being complete or mental capacity assessments not being completed, had not been previously identified. This shows a lack of effective quality assurance systems in place . The service was completing monthly resident of the day reviews, which included review of care plan and care records, however these reviews had not been effective in identifying the issues found during this inspection. This was a breach of regulation 17 (Good governance) of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014.

Learning, improvement and innovation

Staff told us they felt well supported by their management team. No concerns were raised on inspection about the management team.

During this inspection, we reviewed notes from a visit completed by the local authority quality team in January 2024 highlighting some of the issues we found during this inspection such as issues with medication and recording of mental capacity assessments. However, we continued to find these issues on our visit and we did not see evidence that the provider's action plan had been updated to show progress and timescales to address known issues. Although the manager told us the quality assurance processes that they had put in place, during this inspection we found these had not always been effective in identifying and addressing continued issues we found. This was a breach of regulation 17 (Good governance) of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014
