Safe
The manager had continued to make improvements at the service and had clear processes in place to enhance a positive learning culture. They and their staff worked to support people effectively when they required support from external health professionals.

Learning culture
Relatives we spoke with were able to give examples where staff had learnt from incidents to provide safe care for their family member. One relative told us their family member had developed a pressure ulcer some time ago. They told us staff had made changes to their family member’s repositioning regime and had taken advice from district nurses. There had been no issues since then. The relative went on to say they could approach the managers about anything. They said “I am over the moon with the management team in place. They have the knowledge they need to run the home, they know what they are doing.”
Staff were very positive about how they were supported to learn from events. They told us the manager and deputy manager made sure things were fed back at handovers, through supervision and notices in the nurse’s office which staff have to sign to show they had read the information. One member of staff said, “Any changes to people's care is always discussed but the manager is a great one for putting things on notices for us. If we have been on leave then the seniors and the managers will make sure they check back and give you the full information about any accidents or incidents and any changes to people’s care.” The external health professionals we spoke with, were positive about the way the service worked with them and learnt from incidents and events. One external health professionals said, “Yes, we have always found that the staff work closely with our surgery to address and learn from incidents. Whenever an event occurs, they appear to take immediate action and review the situation to identify what could be improved.”
Learning at the service was supported by good processes. There were up to date polices to support and guide staff. The manager and deputy followed clear processes mentioned above, to ensure staff were kept informed and learnt from events.
Safe systems, pathways and transitions
People told us they were supported if they needed to attend any medical appointments. Some people told us they were able to walk to the local GP with staff or relative’s support. But if they needed to attend hospital appointments then the manager arranged for either staff or relatives to support them and ensured they had appropriate transport for the appointments.
The manager told us if people needed readmitting to the service from hospital they would ring the hospital to check if the person’s needs had changed. This would allow them to assess whether they could support the person or if any specialist equipment or skills were needed.
Hospital passports were in place to ensure hospital staff had the information they needed to care for a person in the right way. There were policies in place to make sure staff were aware of how to safely support a person’s admission to hospital.
