Effective
People told us they were not always involved in their care planning and we found care plans were not always clear if people had capacity to make decisions about their care and support and how this had been communicated. People’s comments included, “I know nothing about a Care Plan” and “I know nothing about a Care Plan, I don’t know anything about any reviews”. The registered manager confirmed a proportion of care plans were reviewed each month and others could be identified as needing to be reviewed. Some improvements were required to updating care plans and involving people with this. People received visits from district nurses and were supported with advocates, GP’s and social workers. Positive feedback was provided by two professionals we spoke with. They confirmed they were kept up to date with information and the support people received was good. People were encouraged to remain independent with undertaking daily tasks such as personal care and getting their meal from the dining area. Another person told us how they had a bath by choice and that they had a kettle and fridge in their room. They said, “I have a bath by choice (and) I have a fridge and kettle in my room”.

Assessing needs

We found care plans were not always clear if people had capacity to make decisions about their care and support as it was not always recorded. There was support from health and social care professionals. When we spoke with two professionals they confirmed people had advocates and that the person was being supported with their individual support needs. People had a Dols in place which did restrict some access to the community. Positive feedback was provided by two professionals. People were encouraged to remain independent with undertaking daily tasks such as personal care. People were supported to access the community.
Staff knew people well and were able to tell us how they supported people with their individual risks and needs.
Care plans contained good information about people’s history, their routines, likes and dislikes. However, there was limited guidance for staff in how to support the person with personal care, if they had capacity to make decisions about their care and support and risk assessments were not always in place giving staff important information in how to support people to remain safe.
