Responsive
Staff told us that care plans were devised and reviewed by the multidisciplinary team, and that patients received personalised care and treatment. However, not all patients who were able to, had not been involved or seen their plan of care. The provider had a designated physical health team in place, who contributed to regular monitoring for certain physical conditions, and staff implemented relevant care plans around individual physical healthcare needs. Staff told us that the loss of ward based social workers had impacted on patients, including the facilitation of discharges, liaison with external safeguarding teams and liaison with the Ministry of Justice.

Person-centred Care
Five patients told us that they had not been involved or seen their plan of care. Two patients told us that they were not supported to make changes, one patient told us that their care was not patient centred, and another patient told us that they could not get access to information.
Staff told us that care plans were devised and reviewed by the multidisciplinary team, and that patients received personalised care and treatment. Staff told us that the loss of ward based social workers had impacted on patients, including the facilitation of discharges, liaison with external safeguarding teams and liaison with the Ministry of Justice.
Patient’s care plans fully reflected their physical, mental, emotional and social needs, including those related to protected characteristics under the Equality Act. Most patients who used services and those close to them (including carers and dependants) were involved in planning and making shared decisions about their care and treatment. The provider had a process in place for patients to be involved in the planning of their care and undertook both regular care plan and care coordinator audits. However, five patients told us that they had not been involved and two patients told us that they had sometimes been involved in their care planning.
There was a process in place for patient's to be involved in their own care planning, however there was limited evidence of patient involvement. Staff had not clearly documented when patients had been involved in their care planning process, or when they were unable or not willing to engage. There was a designated physical health team, who contributed to regular monitoring of certain physical conditions, and staff implemented relevant care plans around individual health needs. The provider undertook both regular care plan and care coordinator audits.
