Governance, management and sustainability

Staff that we spoke with understood their roles and systems were in place to monitor patient outcomes, staff performance and relevant changes to best practice guidance.

Leaders used effective governance processes to monitor risks and outcomes, and to drive improvement. Leaders undertook a range of standard reviews and audits to monitor performance. The results of these were discussed at governance meetings. Leaders shared updates and learning with staff through a range of communications. Policies and procedures reflected this and organisational performance was benchmarked against other services to measure outcomes of care and treatment. We saw examples of deep dive investigations when themes and trends had been identified such as falls so that the service could learn lessons. A risk register for the service set out key risks as well as actions and mitigations and environments, workforce and financial strategies and key reports kept leader informed of challenges faced and short, medium and long term plans. Examples of supporting reflective practice were provided by the service and leaders told us how softer language was used to support a culture of learning and not blame. Service level agreements with 3rd party providers were regularly monitored and reviewed. We saw an example of a community consultant agreement.
