Assessing needs

People and family members were aware of care plans and were happy with the way care was provided. One family member told us, “Yes, I have discussed them with the manager and seen her care plans.” They added, “I get informed of any changes. Yes, I am involved in review meetings, she is due one soon.” Another family member said, “I haven’t seen her care plans for a while but did in the beginning.”

The registered manager described the pre-admission process which included seeking information from the person, family members and any professionals involved in the person’s care. They identified this helped ensure a person-centred service and that only people whose needs could be met at Woodside Hall would be admitted. Nursing and care staff confirmed they were provided with all necessary information about new and existing people to enable them to meet their care and support needs.

The provider had suitable policies and procedures in place for the admission of new people which should ensure that only people’s whose needs could be met were admitted. Following admission nursing staff developed individual care plans for each person. There were some inconsistencies within care plans/risk assessments. However, this was already known to the provider and registered manager following internal audits and they had actions in place to work with nurses to address this.
