The assessment was carried out between 7 March 2024 to 21 March 2024. Withins (Breightmet) Limited is a residential care home providing personal care for up to 65 people in one purpose-built building over three floors, including autistic people and people with a learning disability. We carried out this assessment to review the service's response to a safeguarding incident and concerns we had received around infection prevention and control and staffing. The inspection was also prompted in part by notification of an incident following which a person using the service sustained a serious injury. This incident is subject to further investigation by CQC as to whether any regulatory action should be taken. As a result, this assessment did not examine the circumstances of the incident. However, the information shared with CQC about the incident indicated potential concerns about the management of environmental risks. We received positive feedback from people living at the service. People spoke positively about staff and the help and support they received. Medicines administration was not always managed in line with best practice. At the last inspection at this service we identified some concerns around the safe management of medicines. During this assessment we found similar concerns around 'as required' medication . We found that all prescribed medicines were not being offered to people as prescribed. This should have been discussed with the GP when there were concerns that the medicines were not required. This had not been done. Following the assessment, staff contacted the GP to review if these medicines were still required. We found the service had acted appropriately in response to the safeguarding concern raised. Care plans did not always contain sufficient information to support people with their individual risks and clearly guide staff on how they would like to be supported. There continues to be a breach of regulation in relation to medicines.
