During a routine inspection
People were given appropriate information and support regarding their care and treatment. Health Action Plans (HAP) and Person Centred Plans (PCP) were in easy read accessible formats so that people understood them. Staff would verbally go through the plans with people.
Care plans and assessments of people's capabilities were carried out annually and updated in between if a person's needs changed. We found that the home was updating these documents in line with the provider's own guidance.
The provider had an effective system in place to identify, assess and manage risks to the health, safety and welfare of people using the service. For example Yarrow's head office carried out spot checks to ensure the home were meeting standards across a range of areas i.e. care planning. The provider also had arrangements in place to identify staff training needs and other areas staff needed support with.
