Although we found good practice at this assessment, until we have assessed more quality statements for this key question, the rating cannot improve beyond requires improvement. People had the best possible outcomes because their needs were assessed and plans were in place to ensure their care was effective. The support people received from staff reflected their needs and any protected equality characteristics. Leaders instilled a culture of improvement, where understanding current outcomes and exploring best practice was part of everyday work. There was a new manager in post at the time of our assessment. They were getting to know people and worked with the staff team to review how care was being delivered to ensure it was effective.

Assessing needs

People spoke positively about the care they received from staff who understood their needs well. People told us staff supported them to access health and social care services where needed.

It was evident from our discussions with staff that they knew people well and understood people’s needs.

The systems and processes in place ensured people’s care plans and assessments were up to date. Assessments considered a range of needs and contained some helpful information that was personalised and reflected people’s current needs. People’s care plans were reviewed regularly and also when people’s needs changed.

Delivering evidence-based care and treatment

People told us they were involved in making decisions about how their care needs were met and staff worked with them to identify what was important to them. We observed a lunchtime experience. People chose where they would like to sit, and what drink they would like. Some people had changed their mind about their pudding choice, so staff offered an alternative.

Staff told us they had received additional training to help ensure the way they supported people followed current evidence-based good practice and standards. Staff told us their practice was checked by the provider and they felt any additional training needs they had would be met by the provider and new manager.

The provider had improved their assurance processes to ensure people received care and treatment that was appropriate and up to date.

How staff, teams and services work together

People told us staff now worked well across the team and they felt communication had improved.

Staff were positive about the improvements that had been made and told us they had benefitted from further training. Staff spoke encouragingly about the new management structure and reported staff morale had improved. Staff stated they would like to see further team working embedded and the new manager was keen to ensure this happened.

The provider and new management team had implemented more robust processes to ensure information was shared appropriately with internal teams and external professionals, where necessary. Staff team meetings were held more regularly to ensure updates could be provided consistently to all members of the team.
