We carried out an assessment on Church Farm Care Home from 02 October 2024 with an unannounced site visit carried out on the 16 October 2024. This was due to the services last inspection rating; to ensure that they had implemented improvements and that these were beginning to be embedded within the service. We noted a significant improvement in staff morale and the environment was more suited to people living with Dementia and there was an ongoing improvement plan to refurbish the environment. We found staff on the day of the onsite assessment were proactive and when issues were identified they were acted on immediately. However, the processes still required a lot of improvement, there were inaccuracies in care records and risks were not clearly identified in risks assessments or mitigated. There was improvement required around staff training and competency and we were not assured the provider had clear oversight within the service due to a lack of processes and new processes that were implemented we were not assured they were fully embedded. We spoke to 10 staff within the service to obtain their feedback.
