People’s needs were assessed on admission and regularly reviewed. Appropriate people, families and other stakeholders were involved in reviews where this was appropriate. Information about people’s changing needs were shared with staff during daily handovers and staff had easy access to information about people in their care plans.

Assessing needs

People were not always able to state how they had been involved in the assessment and care planning processes. One person commented, “I'm pretty certain my [family member] will have been involved.” People were overall happy with the care and support they received. One person told us, “[Staff] they tell me what they are doing……the care is very good.”

Nursing staff were pivotal in ensuring care and support to people was relevant and appropriate. Care plans were formulated from the outset and added to following changes in need, for example due to a deterioration in health, an accident or a new medical condition. Staff understood and followed the processes for assessment, review and care planning. Partner agencies told us people were appropriately placed at the home. Regular reviews of care took place with commissioning partners to ensure people’s changing needs were recognised and met.

An assessment of people’s needs was undertaken prior to people coming into the home to ensure they received safe and effective care and achieved the best possible outcomes. These assessments considered the person’s health, existing health conditions, past medical history, medication, wellbeing, and communication needs, and from the information gained from people and relevant stakeholders an electronic care plan was created. This care plan informed care staff what care and support the person needed and how they preferred this to be delivered. Assessments were up-to-date and staff understood people’s current needs. People’s care needs were routinely reviewed by nursing and senior clinical staff, with any necessary changes being made and communicated to care staff. Input from external professionals, such as dieticians and occupational therapists, were also recorded onto electronic records so care plans fully reflected all care and treatment.
