Our view of the service

Hampton Grange Nursing Home is a residential care home providing personal and nursing care for up to 42 people within one large, adapted building. It specialises in the care of people living with dementia and older people requiring general nursing care. At the time of our inspection, there were 29 people living at the service. We undertook an assessment of the service on 10 and 12 April 2024. We inspected this service to follow up on our last inspection and due to concerns reported to us. The concerns included poor nutrition and hydration, and poor record keeping. We assessed 4 quality statements in the safe key question: safeguarding, involving people to manage risks, safe environments and medicines optimisation. We assessed 1 quality statement in the effective question: delivering evidence-based care and treatment. We assessed 3 quality statements in the well led-key question: governance, management and sustainability, partnership and communities, and learning, improvement and innovation. We found areas of concern relating to record keeping and acting upon behavioural incidents. We identified 1 breach of regulations. This inspection was carried out by 2 inspectors and 1 specialist advisor who specialised in nursing. The scores for this area have been combined with scores based on the key question ratings from the last inspection. The overall rating for the service remains requires improvement.

People's experience of this service

People gave us mixed reviews of the service. People were content with the clinical aspect of running the service, however, some people raised concerns with us about the management of other people’s behaviours. One person told us, “It happens often, but it's not too bad, that someone wonders about and walks into my room. Sometimes I'm scared but they don't know what they are doing”. Records relating to behavioural incidents were not always of good quality, with some key information missing. This meant that people’s relatives and other professionals did not always receive a full picture of circumstances around notifiable incidents. Some people told us their teeth were not always brushed. We checked people’s toothbrushes and found that 10 people’s toothbrushes were dry on the first day of the inspection. We raised this with the service provider and on the second day of our inspection we found no dry toothbrushes and people did not raise any concerns regarding the oral care they had received. People were given appropriate food and fluid and received their medicines as prescribed. Whilst living at the service, people were able to access other healthcare professionals when they needed to. People had their nutritional needs assessed by the service. People who were identified to be at risk of malnutrition or dehydration had appropriate monitoring charts in place. However, their food or fluid intake was not always recorded. People received their medicines as prescribed. Where necessary, people were referred to other healthcare professionals, such as a GP, a speech and language therapist (SALT) or a mental health team.
