Date of assessment 9 October 2024 to 26 November 2024. The inspection was prompted in part by notification of an incident following which a person using the service was harmed. This incident is subject to further investigation by CQC as to whether any regulatory action should be taken. As a result, this assessment did not examine the circumstances of the incident. However, the information shared with CQC about the incident indicated potential concerns about the management of risk of safeguarding people from abuse. This inspection examined those risks. This assessment was also prompted by concerns about staffing and risk management raised with CQC by the Local Authority. At the previous inspection in March 2022, the service was rated Requires Improvement and we identified 8 breaches. This assessment identified continued breaches in relation to safe care and treatment, good governance, staffing and failure to notify CQC of other incidents. People did not always experience safe care as information, appropriate guidance and support was not always accessible. There were not always enough skilled staff to deliver safe care to people. People were at risk of unsafe moving and handling. People did not always have access to drinks. We assessed a small number of quality statements from the safe and well-led key questions and found areas of concern. The scores for these areas have been combined with scores based on the key question ratings from the last inspection. Though the assessment of these five quality statements indicated areas of concern since the last inspection, our overall rating remains requires improvement. We have asked the provider for an action plan in response to the concerns found at this assessment. In instances where CQC have decided to take civil or criminal enforcement action against a provider, we will publish this information on our website after any representations and/ or appeals have been concluded.
