Effective
People’s needs were assessed and their wishes central to the care, support and treatment they received. Staff worked with people and partner services to coordination care and improve their experience of services. Staff we spoke with onsite told us how they learnt from their own and practices of others. They used this information to inform and make positive changes to how they delivered care individually and ultimately collectively. Their central objective being to improve efficiency within the service and patient outcomes such as improving a patient’s quality of life and/or their ability to manage their conditions.

Delivering evidence-based care and treatment
Clinical staff did not receive formal supervision to ensure their decisions were reflective of national legislation, evidence-based good practice and required standards.
There were not established and effective systems in place to identify, share and embed best practice. Audits had not been conducted on administrative processes such as two week wait cancer referrals or safe prescribing. Regular management meetings were not held. Risks were documented for discussion for example, poor clinical practice, changes to ways of working and delays in processing clinical referrals. However, no further updates were given, or resolutions stated at subsequent meetings.
