During a routine inspection
Comments included 'The service is excellent' and 'I am really happy with my key worker. They are very helpful and understanding. They do an excellent job'.
We saw that risk assessments were not always regularly and appropriately reviewed. For example one person who used the service had reported an increased change in their illicit substance use which they were using on top of their prescription. Their risk assessment had not been reviewed as a result of this information or any record made of any discussions had with the person in relation to the increased risk of overdose and harm for example.
We saw meeting and review records that demonstrated staff supporting people who used the service contacted other relevant agencies involved in people's care and support; for example midwifes, GPs and prisons. However we did always not see up to date records that explained what involvement the other agency/service had.
We saw documents which confirmed that all safeguarding issues were now recorded as individual incidents and regularly monitored by the provider's safeguarding lead as well as the Clinical Incident Review Group.
We observed that there were sufficient staff throughout our visit to provide services to people dropping into the service as well as those attending appointments. We saw that people who used the service were greeted with respect and courtesy.
The provider ran two groups for people who used the service that provided them with an opportunity to comment on the quality of service provision; the Recovery Task Force and Service Users Forum. We saw that these meetings had been recorded and action taken as a result of feedback received.
We noted that individual client case management was discussed in both staff supervision and weekly Multi-Disciplinary Team meetings. We saw that these had been recorded however information pertaining to specific clients was not then updated on their personal records.
