Well-led
The Trust had implemented measures to address the issue we had identified during our previous inspection. We found that managers had an audit schedule in place and audits highlighted areas for improvement.

Governance, management and sustainability
At our previous inspection we were informed audits were completed. However, they had not identified the issues with patient care planning, patient records and managing risk to falls. For this assessment we were provided with an audit schedule. This provided detail as to which audit managers needed to conduct month by month, and the outcomes for each audit. Audit reviews were undertaken by a number of senior managers within the Trust, to ensure that audits were effective in picking up areas for improvement in patient safety. Regular audits were now undertaken to ensure that staff were following procedures correctly. We were sent the Local Operating Procedure for, managing medication changes. Managers carried out audits to ensure staff were following the procedure to ensure there were clear records for the decision-making process when patient medications were changed, and patients were aware of the reasons for any amendments.
