Responsive
The end of life care service comprised of a specialist palliative care nursing team and community nursing teams. Medical support was provided by a service level agreement with a local NHS Trust. Care and escalation was overseen by the community nursing teams which was evidenced when tracking patients. The service had a good understanding of the local population and responded to the needs of people and making reasonable adjustments to support equity in experience, pain and symptom management.

Care provision, Integration and continuity
Patients told us they felt staff responded to their needs from the community nursing and specialist palliative care nursing team.
Staff told us they worked collaboratively with other services to respond to the patients deteriorating needs to ensure they were comfortable and their wishes were met where possible. This included therapy staff, arranging additional equipment such as beds, commodes' and reviewing medicines. Leaders told us they worked closely with the local hospital and hospice to provide continuity of care.
We did not request feedback from partners for the end of life care service.
There were clear systems and processes for monitoring and reviewing patients on end of life care. The service had pathways between other organisations to provide continuity of care. We saw staff supporting and resolving issues for patients between services. The community nursing teams followed clear pathways for escalating patients deterioration and pain management. There was a referral process for the community nursing team to refer patients' to the specialist palliative care nursing team for support, but this was not an emergency service. Any emergency was dealt with by the community nursing teams and medical staff.
