Effective
People had not always been involved in assessing or reviewing their support choices or needs. Care plans were in place, but these were not always consistent or regularly reviewed. Support to people with eating and drinking had improved, though care plans did not always reflect this.

Assessing needs
People told us they were involved in decisions about how they were supported. One person said, “I get a say in what I do and how it’s done.” However, people’s feedback had not been incorporated into care plans.
Staff were knowledgeable about people’s support needs and preferences. Staff we spoke with knew people well. However, care records did not always reflect this knowledge.
Care plans were not always consistent in recording people’s needs and decisions. Reviews of plans did not take place regularly, and where they did people had not been involved. In some cases, reviews had not identified changes in people’s support needs.
Delivering evidence-based care and treatment
People spoke positively about eating and drinking at the service. One person said, “Breakfast, dinner and tea. Always a consistent routine. Food is always delicious and hot.”
We saw that staff worked collaboratively to ensure people received the support they needed with eating and drinking. Communication between kitchen staff and care staff had improved since our last inspection.
Processes and records for monitoring people’s weight and nutritional health had improved since our last inspection. However, further and sustained improvement was needed to the cohesiveness and consistency of care plans. For example, for some people we saw that their plans did not match practices by staff. We spoke with the registered manager about this, who said plans would be reviewed. People had access to external professionals where needed, and we saw one visiting the service during our visit.
