People’s care plans were personalised and reflected their needs and preferences. Risks to people were identified and recorded so safety was maintained. People were supported to have choice and control and were involved in planning their care. People were referred to other health agencies to promote their wellbeing. People were provided with information they could understand. People knew how to give feedback. Managers were visible and supportive. Staff understood their roles and responsibilities. Systems were in place to monitor the quality of the service and care provided. Actions were taken to continuously improve the service.
