Responsive
The provider had systems in place to listen and respond to people and their relatives. Staff understood the principle of person centred care and peoples care plans reflected people’s needs and preferences.

Care provision, Integration and continuity
Everyone we spoke to told us they received good care. They told us they were offered support and information, and health professionals were involved, to ensure their health and wellbeing was managed effectively.
Staff and leaders worked in partnership with external professionals when required and followed their advice and guidance.
We received feedback from a number of partners including local authority. All the feedback we received was positive and described a picture of an engaging service.
The service had processes in place to ensure the effective engagement and availability of local primary care services. The service understood it’s model of care and made robust assessments of people to make sure their needs could be met. They involved local primary care services within these assessments, including GPs, speech and language therapists, local opticians and a chiropodist service.

Equity in experiences and outcomes
People were supported to live their lives in the way they wanted. Staff respected people’s beliefs, choices and protected characteristics.
Staff had received dignity, equity and diversity training, and we saw evidence of discussions in supervision regarding training attended and learning into practise. Staff showed a good understanding of possible discrimination and inequality. Although they shared no concerns currently within the service, staff were confident they could report concerns and action would be taken.
The service had information for people available in a range of formats. The design of the environment supported people to navigate it easily. The management of the service were aware of inequalities and actively promoted the involvement of people, relatives and when appropriate advocates to ensure assessments and care plans reflect any risk of inequality. Care plans were reviewed to make sure they were in line with people’s wishes and met equalities/protected characteristics.
