People using the service, and their relatives told us they felt safe living at the care home and that any concerns were managed promptly. Relatives reported that the service continues to improve & they have seen a positive difference in their relative’s health and wellbeing. Staff demonstrated a good understanding of people’s needs and risks. For example, staff knew who needed repositioning and how frequently, who was at risk of developing pressure sores and what support people needed in relation to eating and drinking. Risks were identified in individuals care plans and any changes were shared within the staff team to help keep people safe from harm. We observed staff supporting people to remain safe in the home. This included supporting them with their mobility, personal care & eating and drinking. We observed staff supporting people to sit in an upright posture whilst eating as identified in their care plan. Staff followed guidance around the use of supportive and pressure relieving equipment and we observed safe moving and handling practices. Staff engaged well with the people using the service & were very friendly. Risks identified were monitored, reviewed and shared with staff to ensure there were appropriate risk assessments in place. These plans covered every aspect of people’s lives including, mobility, skin integrity, nutrition & dehydration, and other health needs such as Diabetes and Epilepsy. Staff undertook mandatory training and additional training relevant to people’s needs including diabetes, epilepsy, skin integrity & tracheostomy. Staff worked in partnership with other professionals to incorporate their advice and guidance to effectively manage risk. There were systems in place for staff to report concerns, incidents, and accidents. This enabled the provider to have oversight and manage risks. The management team completed action plans devised from audits and their own observations to minimise risk and keep people safe.

Involving people to manage risks

Score: 3

People and relatives were positive about the care they received at the service. They told us they were involved in developing and reviewing their care and felt consulted about how risks were managed. Comments included, “We always know what’s going on, have been through [my relative’s] care plan and checked it”. Relatives told us that risks in relation to their family members health and medical conditions were managed effectively. They told us that there had been improvements in their family members health through support from staff. Comments included, “[My relative] uses a frame to move to a chair, it’s such an improvement. [My relative] has progressed” and, “I couldn’t have wished for a better place. [My relative] has gained weight and no longer looks gaunt, she had sores but now she has none."

Staff were confident in their role. They demonstrated a good understanding of people’s needs and how risks should be assessed and monitored. Staff told us they had received enough training relevant to their role and that they were supported by senior staff to promote their learning as people’s needs changed or their medical conditions progressed. The registered manager had a good oversight of the service and regularly monitored people’s care records to help ensure care was carried out as planned. They held reflective meetings with staff when issues or mistakes were identified. This helped to promote a positive learning culture and robust risk management.

We carried out observations of staff providing care to people in communal areas. This included support with moving and handling, eating and drinking and repositioning. We observed staff were confident in carrying out these care tasks, supporting people in a dignified and person-centred way. We observed that staff followed specific guidance in relation to managing people’s health and medical conditions. For example, staff followed guidance around the use of pressure relieving equipment and we observed safe moving and handling practices. Where people had been assessed to require food and fluid prepared to a specific texture, staff were knowledgeable about people’s individual requirements. This helped ensure risks related to eating and drinking were minimised.

There were effective policies and procedures in place to assess, monitor and reduce risks. People’s care plans and risk assessments were reflective of their needs, incorporated professional’s guidance where relevant and were regularly updated as people’s needs changed. The registered manager had a good oversight of people’s care. The provider had an electronic planning system, which helped to identify key tasks in relation to people’s care and alert the registered manager when care was not carried out as planned. This helped to ensure that any discrepancies were quickly recognised and addressed to minimise the impact on people’s health and wellbeing. The registered manager completed audits of people’s care plans, care records and call bell response times to ensure that care was being carried out effectively and as planned. The registered manager shared learning outcomes from audits with staff to promote safety and robust risk management. This helped to ensure that staff were working in a consistent and safe way when supporting people.
