At our last inspection we rated this key question good. At this assessment we did not assess all quality statements within this key question. Assessing needs

People, relatives where appropriate, and health professionals were consulted with when needs were assessed. One person told us, “They (staff) all understand me very well and know what I need so I am very happy.” Another person said, “I do have a care plan and all the staff are very knowledgeable about my condition. They all seem to be very well trained and know exactly what to do if I am not well. They bring all the equipment and are there to help me.” A relative told us, “[Person’s name] has a full care plan which we are involved with and they (staff) always make sure this is updated if anything changes.” Another relative said, “A care plan was developed initially with the support of [person name] mental health worker, social worker, GP and care home manager. We were kept fully in the loop and still are.”

Care plans were in place. Care plans included information on communication, health, mobility, nutrition and hydration, daily lifestyle and emotional support and mental capacity. Some care plans lacked detail. One care plan had limited information and had sections which were not completed. We discussed this with the management team who assured us they would review this as a priority. Staff knew people well and understood their individual care and support needs.

We observed staff asking and talking with people before caring out any tasks to support them. People appeared to be comfortable and interacted well with staff when having their needs met. A relative said, “All care is very individually tailored and delivered with each person’s consent, we really couldn’t ask for better.”

Systems were in place to follow decision making principles on behalf of people who lacked capacity. Staff had received training in the mental capacity act. They supported people to make their own decisions wherever possible, and staff respected people’s choices. However, we identified gaining consent was not always recorded and reflected. For example, people’s care plans only offered limited information about people’s daily decision making.
