Effective
People and their relatives were involved in assessments of care, and relatives felt well informed about the care being provided. Resident of the day meant peoples care was identified and discussed for a named person each day, but this was not always completed thoroughly which meant there were missed opportunities to engage with people. The principles of The Mental Capacity Act (2005) were being followed and people were not necessarily restricted unless agreed in their best interest. We found however peoples care plans were not detailed enough to ensure staff knew how to support people in line with their needs and best interests'

Assessing needs
People's needs were assessed prior to moving into the service and should their needs eventually outweigh what the service could provide people and their families were supported to move on with the necessary support and advocacy if required. People and their relatives felt this was a good service with a range of activities, relatives felt people were safe and that staff followed up on any change to people's needs.
The manager agreed that gaps in record keeping were identified at the time of our inspection and assured us that staff were continuously supported to develop their professionalism and record every thing carefully. Despite poor record keeping we did not identify any negative impact such as service acquired pressure ulcers and the manager was quick to address any issues. For example it was brought to their attention that a number of people appeared to be losing weight and this was addressed holistically by reviewing records, whether supplements were necessary and reviewing the catering.
Peoples care was ongoing and assessments developed over time. care plans were in place and reviewed but sometimes records were archived and we could not see a clear overview of people's needs or how risks were always responded to. For example a new admission food and fluid chart was in place but after a few days was archived and in this instance the person continued to have low food fluid intake and we were unable to establish if this was a normal pattern of behaviour of indicative of a change in need. However the manager told us there was senior management oversight of weights and nutrition which was tracked and shared with other heads of department.

Monitoring and improving outcomes
People had access to health care and were supported to access appointments. Assessments were completed if required and equipment supplied to help reduce falls and specialist diets were accommodated.
Regular surgeries and head of department meetings were used to discuss and escalate risk and ensure peoples health care needs were met in a timely, coherent way . Health care professionals were complimentary and felt that the home utilized other service for the good of people living there.
