Staff and leaders told us they were continuously learning, to innovate and improve. Evidence of learning was seen within practice meetings notes.

We found staff were encouraged to audit and learn from the findings to innovate and improve services and patient outcomes. Evidence of learning was seen within practice meeting notes when discussing complaints and significant events. For example, GPs were reminded to check referrals were sent following a complaint about referral delay.
