People’s care and treatment needs were assessed. People reported high levels of satisfaction with the health professionals reporting they gave them enough time, listened to them, treated them with care and concern and ensured they were involved in decisions about their care and treatment. There were effective systems and processes in place to ensure peoples individual health, care, well being and communication needs were considered by all staff. Additional support was available to people to remove or mitigate barriers to them accessing and engaging in care and treatment.

Assessing needs

People receive care and support from trained staff utilised a range of triage tools to ensure their needs were identified and understood. The practice has invested in improving the knowledge and skills of their reception and dispensary team to contribute actively to a patient centred approach to care. Staff were trained to signpost patients to primary and voluntary care services to support them to access and receiving appropriate care. For example, staff utilised the 111 symptom checker and working with veteran services.

Staff recorded, assessed and referred people to appropriate people to meet their individual needs or delivered shared care in partnership. Staff were trained in social prescribing signposting people to wider health and social care provision where appropriate to support them.

There are established and effective processes for assessing peoples, clinical, welfare and communication needs. There are established care pathways to aide timely and appropriate referrals to specialist services. Clinicians utilise templates to focus assessments and ensure consistent recording of information. The practice has established and effective systems in place with primary care partnership services (local pharmacy, the local dentist, the community nursing, midwifery, palliative care and heart failure teams) to assess and meet patient needs.
