Equity in experiences and outcomes
There was a lack of involvement from people and their representatives in designing and reviewing care records to support their independence, choice and control. Care plans and risk assessments were limited as they did not include people’s preferences.
Staff and leaders told us they knew people well, understood their needs and requirements and promoted these preferences. However, knowledge around people experiencing inequity in care needed developing. We suggest the provider seek advice from a reputable source
