Our view of the service
Date of assessment 26 March to 7 May 2024. Gloucestershire Hospitals NHS Foundation Trust provide a maternity service for women in Gloucestershire. We conducted an evidence-based, desk top assessment of the hospital. The assessment also included an onsite visit on 26 March 2024. This assessment was to follow up on actions we told the trust to take in a previous inspection and in response to concerns raised with us more recently. We assessed a total of 5 quality statements from the safe and well led key questions and found areas of concern in governance and staffing. The scores for these areas have been combined with scores from the last inspection. We found 2 breaches of legal regulations in relation to governance which impacted on care and treatment. Governance systems and audits were not effective in identifying or addressing areas for improvement. Arrangements for ensuring agency staff were familiar with trust systems were not always effective. However, staffing numbers had improved and gaps were filled with agency staff. Staff training in safeguarding people had improved. The assessment of these 5 quality statements indicated areas of concern since the last inspection, although there had been some improvements, our overall rating is inadequate. We conducted a responsive risk-based assessment of the emergency department at Gloucester Royal Hospital which included an onsite visit on 12 December 2023. This was to follow up on concerns raised with the CQC. We assessed a total of 2 quality statements from the safe key question and found areas of concern around fire safety, staff fire safety training and equipment maintenance. The scores for these areas have been combined with scores from the last inspection. We found a breach of legal regulation in relation to equipment and premises which impacted on care and treatment. Staff were not following national regulations and guidance in relation to fire safety. Medical devices were not being tested in accordance with national regulations and guidance. Not all staff were up to date with their fire safety training.
