Care provision, Integration and continuity

The service provided care to people as inpatients and liaised with community nursing teams should patients choose to die or be treated at home. People we spoke with said care was integrated, flexible and supported choice and continuity. Patients had access to a multi faith room and pastoral support was offered by a spirituality team.

The service understood the diverse health and care needs of people and its local community. Care was joined up between the different organisations and was flexible and supported choice and continuity. The service worked with local organisations and the ICB to improve the quality of care provided to patients.

The hospice collaborated positively with many partnerships. The hospice worked closely with local health partners to ensure patients who are were eligible for services received care. They collaborated frequently with partners within the Integrated Care Board (ICB). There were 7 other hospices within the geographical patch who worked closely with Lindsey Lodge to share learning and improve services. There were also steering groups, university based research and quality improvement programmes all working together for the benefit of patients.

Lindsey Lodge operated as a charitable organisation with limited NHS funding. The hospice had a 16-person inpatient unit for when service users required monitoring for a change in their treatment or for when they were end-of-life. Specialist palliative care and symptom management was also provided for day patients. The bed occupancy fluctuated dependent on demand. They had a high capacity of bed fulfilment than comparable hospices. At the time of assessment, the bed occupancy rate was 85%. There was twice daily meetings known as huddles to ensure the smooth transition of appropriate admissions. Care was discussed at weekly area-wide team meetings involving specialist staff from the community, local hospitals, and at internal multi-disciplinary meetings. A hospice liaison nurse undertook visits to local hospitals and into the community to review patients. Early identification of patients in need of hospice care prevented acute re-hospitalisation of patients and facilitated safe transfer of care.
