Effective
There were effective systems and processes in place to ensure people’s needs were assessed and understood. Care plans were detailed, and person centred. Staff were trained to meet people’s needs.

Assessing needs
Care plans provided information about how people wished to receive care and support. The care plans described people’s needs in a range of areas including personal care, and daily living activities. Care plans detailed people’s health needs and how to meet them using recognised tools.
People and their relatives told us they were happy with the care provided. People and their relatives were appropriately engaged in assessing, planning and agreeing the care and support they received.
Staff told us care plans provided information on how to support people and what was important to them and that updates were shared at handovers. Staff we spoke with were knowledgeable about the risks associated with people’s care and treatment and these were frequently reviewed. Staff felt confident with meeting peoples needs and told us they had received training to support them. The registered manager told us how they supported people and maintained contact with health professionals if they had any concerns, or if people would benefit from equipment to assist them with their daily living. They had recently introduced keyworkers to assist updating care plans and sharing information with families.
