People were involved in planning their care to meet their individual health and support needs.

Care provision, Integration and continuity
We observed the activities in the service to be inclusive and people and their relatives gave us positive feedback on the opportunities on offer. One relative told us, “There are lots of activities, [person] really enjoys these.”
The manager had developed systems to aid the continuity of care between Eden Holme Care and local healthcare services to aid timely outcomes when people required healthcare interventions.
Partners described how improvements had been made through collaborative working. One visiting professional told us, “Measures are in place to support staff in developing the skills they need to provide safe and effective care”.
The provider sought feedback from all stakeholders to support improvements at the service. Improved systems and procedures were implemented to promote best outcomes for people.

Equity in experiences and outcomes
People were supported to access the services they required to meet their needs. One relative told us, “I discussed the issue with staff and arrangements were made to get it sorted”.
Staff linked in with local groups which supported people’s wellbeing. For example, the service had worked with an adapted cycling group which enabled people to regularly access the local area.
The provider implemented systems to ensure people, and their families were involved in making decisions about their care. The provider accessed specialist services where required, for example, to support a sensory need.
