Well-led
We identified two breaches of legal regulations. Notifications were not always being made when required including when incidents had occurred and when concerns of a safeguarding nature had occurred. We also found governance arrangements were not always identifying shortfalls found during our inspection this included identify actions needed when safeguarding concerns were raised and making relevant referrals along with improvements needed to some people’s care plans and risk assessments and mental capacity assessments. Families spoke positively around the support provided by support staff and how approachable the registered manager is. One family member told us, “Registered manager is a lovely lady, very positive”. Improvement meetings were being held with external professionals so the service could improve people’s care and treatment and their outcomes.

Families knew the manager and felt able to raise anything with them. Some families had more contact with the service than others. Where families wanted more contact we shared this with the registered manager so they could organise this. One professional shared with us they were working with the service to improve outcomes for people.
The provider had an action plan in place that identified areas for improvement such as improving the quality of handovers between staff and improving gaps in incidents and safeguarding recording although we could not see evidence where the provider monitoring had identified concerns so they were appropriately escalated and actions were taken to minimise incidents from re-occurring.

Governance, management and sustainability
Staff told us there were improvements being made to the service. This included the recruitment of permanent staff and having regular supervision.
Improvements were required to the providers governance arrangements as these were not always identifying shortfalls found during our inspection. Shortfalls included identify actions needed when safeguarding concerns were raised and making relevant referrals including reporting incidents to the Health and Safety executive and following up with relevant referrals and learning outcomes. Along with improvements needed to some people’s care plans, risk assessments and mental capacity assessments, two care plans did not have up to date information relating to their Speech and language assessment. One staff member did not have Epilepsy training. Improvements were also required to notifications being made when required including when incidents had occurred and when concerns of a safeguarding nature had occurred.
