Safe
We reviewed the learning culture, safe environments and infection control quality statements for the safe key question. On the day of the inspection the environment was clean and there was evidence of monitoring of cleanliness through regular audit. Infection Prevention and Control training was low however, we were later supplied with training figures for January 2024 that did show an improvement. During our inspection we found fire doors wedged open, fire exits blocked by stock, and the local fire procedure was not up to date.

Learning culture
We saw evidence that infection control training was not up to date. Local fire procedures and the management of fire exits were seen to be poor, with fire exits blocked and the fire plan not up to date. The environment was clean, and we saw evidence of staff being aware of and implementing the Trust infection control policy, that reflected national guidance, however infection prevention and control training, while improving in January, needs to improve in staff up take.

Safe environments
Visitors informed us of fire exits being locked.
The hospital management confirmed that there had recently been a fire in the department and provided the investigation report.
Local fire procedures and the management of fire exits were seen to be poor, with fire exits blocked, and the fire plan not up to date.

Infection prevention and control
We received feedback from visitors and staff prior to undertaking the onsite inspection, who commented on the state of the department, describing it as dirty, with bodily fluid spills not being mopped up and soiled dressing and other debris left on the floor.
Staff and leaders were able to demonstrate that infection control audits were being carried out and that the unit was regularly cleaned. Infection prevention and control training percentages were low. However, we saw evidence that there had been an improvement in January 2024.
On this unannounced inspection the department was found to be clean and supported by housekeeping staff.
Leaders were able to evidence that audits were being carried out effectively. The trust had completed regular audits and had policies and procedures in place.
