Young people and families were involved in care and treatment planning, reviews and planning for the future, where appropriate. The service worked with partners including the integrated care board and local community teams to aid patient discharge. Patient discharge was sometimes delayed due to lack of community provision. But there had not been any delayed discharges in the last 3 months before this assessment.

Planning for the future

Young people told us they were fully involved in planning for their future. One young person said the ward team worked with social services to ensure a planned placement was suitable to meet their needs and listened to them when they had requested trial leave periods.

Staff told us there was sometimes a delay in the young person’s discharge due to a lack of specialist provision in the community. They told us each ward had an identified case manager from the East Midlands Provider Collaborative who reviewed the young persons progress towards discharge. They also attended a weekly system meeting and the clinical escalation group which looked at complex discharges. They worked with local community teams to improve understanding around the specialist nature of care provided in child and adolescent inpatient wards and to promote safe and timely discharges.

The wards held weekly care plan review meetings where short- and long-term goals were discussed. We saw young people and families were fully involved in planning for the future where appropriate. There had been no delayed discharges in the 3 months leading up to the assessment. The trust held regular provider collaborative meetings, to monitor lengths of stay on the wards.
