Learning culture

Score: 3

The service had a proactive and positive culture of safety, based on openness and honesty. They listened to concerns about safety and investigated and reported safety events. Lessons were learnt to continually identify and embed good practice. There was a positive approach to learning from any accidents or incidents. We saw advice was sought from health care professionals were required.

Safe systems, pathways and transitions

The service worked with people and healthcare partners to establish and maintain safe systems of care, in which safety was managed or monitored. They made sure there was continuity of care, including when people moved between different services. Information in a one-page profile, about the persons preferences and what was important to keep them safe, was available should the person need support from another agency; for example a hospital admission.

Safeguarding

The service worked with people and healthcare partners to understand what being safe meant to them and the best way to achieve that. They concentrated on improving people’s lives while protecting their right to live in safety, free from bullying, harassment, abuse, discrimination, avoidable harm and neglect. The service shared concerns quickly and appropriately. Staff understood their safeguarding responsibilities. They were confident if they raised any concerns they would be dealt with. People told us they felt safe using the service. They said, “Yes, I definitely feel safe” and “I have nothing wrong to say about them.”

Involving people to manage risks

The service worked with people to understand and manage risks by thinking holistically. They provided care to meet people’s needs that was safe, supportive and enabled people to do the things that mattered to them. Risks to individuals and staff were identified and well managed. People told us risks, such as falls, were managed well and they were fully involved in any reviews. One said, “If you have an accident they look after you, they are very understanding of me.”

Safe environments

The service detected and controlled potential risks in the care environment. They made sure equipment, facilities and technology supported the delivery of safe care. Risks to people and staff related to people’s apartments had been identified and well documented.

Safe and effective staffing

Score: 3

The service made sure there were enough qualified, skilled and experienced staff, who received effective support, supervision and development. They worked together well to provide safe care that met people’s individual needs. Staff were recruited safely and received the training and support they needed to carry out their roles effectively. There were sufficient staff deployed to meet people’s needs. People told us there were now regular staff who knew them well. One person said, “I know them now. Its better now there are not as many agency.”

Infection prevention and control

The service assessed and managed the risk of infection. They detected and controlled the risk of it spreading and shared concerns with appropriate agencies promptly. Staff had received training in infection control and they had access to Personal Protective Equipment (PPE).

Medicines optimisation

Score: 3

The service made sure that medicines and treatments were safe and met people’s needs, capacities and preferences. They involved people in planning, including when changes happened. Medicines were managed and administered safely and people told us they were well supported with their medicines. One person said, “ Yes, they are very keen on that.”
