This service scored 75 (out of 100) for this area. Find out what we look at when we assess this area and How we calculate these scores.

Assessing needs

Score: 3

The service made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs with them. A relative told us, “We had a meeting with the team and devised a care plan which we have worked through. This has been updated as [their family member’s] needs have changed.” Care assessments were carefully detailed in people’s care records and provided clear instruction to staff about their needs.

Delivering evidence-based care and treatment

Score: 3

The service planned and delivered people’s care with them, including what was important and mattered to them. They did this in line with legislation and current evidence-based good practice and standards. Staff received regular training and supervision to ensure they provided support in line with best practice guidance.

How staff, teams and services work together

Score: 3

The team worked well together, supporting each other and ensuring clear documentation and communication amongst the team. Staff told us the app they used for recording the care delivered enabled them to effectively share information between themselves.

Supporting people to live healthier lives

Score: 3

The service supported people to manage their health and wellbeing to maximise their independence, choice and control. The service supported people to live healthier lives and where possible, reduce their future needs for care and support. Staff supported people to attend medical appointments when needed. Staff also supported with meals when required to ensure people receive healthy balanced diets.

Monitoring and improving outcomes

Score: 3

Staff regularly reviewed people’s care to ensure it still met their needs and wishes. Relatives told us the service was flexible and enabled them to increase or reduce the level of care their family member required to meet their needs.

The service told people about their rights around consent and respected these when delivering person-centred care. Staff told us, “We would ask them to consent prior to giving care” and “Tell them what I’m going to do. Ask permission. If they don’t want me to do it, then I don’t do it. We ask permission all the time.”
